• Full-time This Facilitator/ICC Coordinator is responsible for the coordination of services to assigned clients and their families.

This position will collaborate with other entities within the SPA and county to ensure maximum level of effectiveness and fidelity to the model.

Responsibilities:

Assists with completion of all documentation and reports, including Child & Family Plans, to ensure all documents meet current and acceptable standards, maintaining fidelity to the Wraparound model and meeting guidelines for all audit entities. Participates in agency-related activities, including, as necessary, the review of charts, client assignments, assessment and programming, crisis response as needed, staff meetings, screening and intake, and internal/external quality assurance activities. Under supervision of the Program Manager of Field Based Services, the Facilitator collaborates with other entities within the SPA and county to ensure a maximum level of effectiveness and fidelity to the model. Ensure maximum participation in the planning, implementing and evaluation of services by clients/consumers through their participation and the advisory councils. Under general supervision, has responsibility for a caseload involving children and/or adolescents and their families; working closely with the resource pool of Child & Family Specialists, Parent Partners, Therapists and any representatives from the referring county department to convene the Child & Family Team Meetings for the purpose of establishing and evaluating the Safety Plans; promotes family decision making and consensus building; provides outreach and emergency services and counseling in crisis situations; performs related work as assigned. In the Field Based Services, Facilitator/ICC Coordinator must adhere to agency and regulatory body standards, policies and ethics for professional roles, conduct and behavior. Serves as a role model for children, adolescents and adults who may have emotional or behavioral problems. Demonstrates appropriate daily behavior, appropriate expression of emotions, as well as appropriate role modeling.

Requirements:

• Bachelor's Degree in the behavioral sciences from an accredited and recognized college/university and two or more years of experience working with seriously emotionally disturbed children and adolescents and their families.
• Experience preferred in working with in-home and/or school-based programs and experience with effective models of child abuse prevention and intervention.
• Obtain Red Cross CPR, First Aid and Pro-ACT certification and maintain such certification throughout the term of employment with SGCC.
• Meet Title XXII personnel requirements with includes but is not limited to: obtaining and maintaining clearance on fingerprints, child abuse index, pre-employment physical, TB test, and drug test.
• Maintain a valid driver's license and provide proof of insurance.
• Bi-lingual skills in Spanish/English preferred.
