Location:
Requires residency in Missoula County or a county contiguous to Missoula County.
Essential Duties and Responsibilities:
Develops person centered recovery plans and periodic reassessment at least once every 90 days to determine service needs, including activities that focus on identification of any medical, educational, social or other services. Participates in the development of RN care plans and reassessments as a member of the HCBS team. Arranges and coordinates services proposed in person centered recovery plans in accordance with accepted standards. Prepares cost estimates for care plans in compliance with waiver requirements. Makes home visits and/or telephone contact as-needed to evaluate service effectiveness and assess continuing client needs. Monitors the quantity, quality and cost of services provided under each care plan. Maintains compliance with the Department of Public Health and Human Services ("DPHHS") and agency requirements for documentation of care. Assists in service coordination to ensure member's health and safety and addressing service and provider issues.
Minimum Qualifications:
Qualifications include bachelor's degree in social work or related mental health field from an accredited college, a valid driver's license, appropriate auto insurance and reliable transportation. 1 year of previous experience in human services and/or experience with adults with mental illness. Working knowledge of community resources preferred. Home & Community-Based Services is a dynamic program that enables older adults and persons with disabilities to live fully integrated lives within their own communities.
