Responsibilities Conduct comprehensive patient assessments to determine psychosocial needs and develop individualized care plans aligned with clinical ethical guidelines and social work practice standards. Provide clinical counseling, including individual and group therapy sessions, utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral health strategies tailored to hospice patients. Coordinate community resources to support patients and families, including assistance with discharge planning, home care services, assisted living options, and bereavement support. Document all patient interactions accurately within electronic medical records (EMR systems), ensuring compliance with HIPAA regulations and medical documentation standards. Collaborate closely with healthcare professionals across disciplines to facilitate seamless end-of-life care delivery while managing case loads effectively through case management practices. Deliver crisis intervention when necessary, addressing emotional distress or behavioral health crises with sensitivity and professionalism. Stay informed about hospice & palliative medicine advancements and maintain certifications such as basic life support (BLS) and CPR to ensure safety and quality care delivery. Requirements Master's degree in social work, with licensure or certification as required by state law for medical social workers. Proven experience working with geriatric patients, individuals with disabilities, dementia care (including Alzheimer's), or those receiving hospice & palliative medicine services. Strong knowledge of medical terminology, ICD coding systems (ICD-9/10), Medicare regulations, and clinical research methodologies relevant to hospice care. Demonstrated ability to conduct diagnostic evaluations, intake assessments, and mental health counseling within a healthcare setting. Excellent client communication skills coupled with the ability to collaborate effectively within an interprofessional team in inpatient, outpatient, home & community care environments. Experience working with diverse cultural backgrounds and individuals from various age groups including pediatrics and seniors is highly valued. Familiarity with EMR systems for documentation purposes and experience in crisis management or acute care settings is preferred. Ability to handle sensitive situations with empathy while adhering to confidentiality standards such as HIPAA.
