Job Summary Join Our New Medicaid Clinic in Lawrence, MA Be part of a growing, mission-driven clinic focused on improving access to care for underserved communities. Our Riverwalk Internal Medicine practice supports approximately 1500 Medicaid ACO patients, providing compassionate, coordinated care to individuals with complex medical and social needs. This is an exciting opportunity to join a collaborative team helping shape a new model of community-based care. About the Role We are seeking a compassionate, resourceful Social Worker to support a dedicated Nurse Practitioner and patient panel long term. This consult-based role focuses on addressing social determinants of health, connecting patients with community resources, and helping patients navigate complex healthcare and social systems. You'll work closely with providers, nurses, and care coordinators in a highly collaborative environment while building meaningful relationships with patients over time. This position offers an excellent opportunity to develop skills in ambulatory care, care coordination, and community-based healthcare, with access to mentorship and professional growth opportunities. Schedule & Location Full-time, 40 hours per week, Monday-Friday (hybrid schedule: on-site at 500 Merrimack Street, Lawrence, MA with remote work on Fridays) Flexible hours of either 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM based on preference and practice needs Why Join Us? Key Responsibilities Perform biopsychosocial assessments and develop personalized care plans Collaborate with medical teams to evaluate patient needs and coordinate services Monitor progress, document outcomes, and adjust treatment strategies as needed Advocate for patients' social, emotional, and environmental well-being Facilitate skill-based groups and participate in quality improvement projects If you're passionate about helping patients navigate complex needs while being part of a mission-driven, community-focused team, we'd love to connect with you.
Qualifications Required:
Master's degree in
Social Work, Mental Health, or Behavioral Medicine Valid Class D Passenger Vehicle Driver's License Preferred:
