00 Student Affairs 1155060 Working/Functional Title Case Manager for Care and Intervention Position Summary Supports, plans, and manages cases of students who are of concern. Provides guidance to other university officials and to students in order to promote health, safety, and well-being of the university community. Responsible for intervention with those community members who may pose a threat to themselves and/or others. Primary function is to manage a case from intake to closure, and to support the work of the Care and Intervention Team.

CHARACTERISTIC DUTIES

• RESPONSIBILITIES Provide outreach to graduate students, undergraduate students, and employees referred to the Care and Intervention Team.

Develops, promotes, implements, and evaluates programs and workshops for graduate students, undergraduate students, and employees. Develops resource and training materials. Connect employees with the appropriate resources on and off-campus. Serves on committee(s) at the University, local, regional, and national level. Provide intake appointments to conduct a holistic assessment of needs. Provide follow-up appointments as appropriate to coordinate action plan, referrals, and support. Connect graduate and undergraduate students with the appropriate resources on and off-campus. Communicate and collaborate with appropriate staff, resources, and the Care and Intervention Team. Document case notes in electronic record-keeping system. Track cases and informs supervisor of current cases. Care and Intervention Team Responsibilities Attends and participates in Care and Intervention Team meetings by having case notes prepared, reviewing new referrals, and contributing to the discussion. Makes referrals to the Care and Intervention Team as appropriate for graduate students, undergraduate students, and employees working directly with case management. Supports the Care and Intervention Team by providing interventions to graduate students, undergraduate students, and employees Serves as a backup to the case manager supervisor on the Care and Intervention Team as needed. Outreach and Awareness Responsibilities Creates marketing, awareness, and education campaigns to promote case management services. Develops and delivers educational presentations tailored to meet the needs of the audience. Coordinate with various campus offices to ensure coherent integration of messaging. Departmental Responsibilities Complete special projects or other duties as assigned by supervisor. Assist in compiling end-of-semester and end-of-year reports. Assist in developing and implementing strategic plan(s). Participates in assessment efforts related to case management effectiveness. Participates in other committees as assigned.

Standard office environment. The job requires very little physical effort. This position is designated as remote-friendly and follows a hybrid schedule. DESCRIPTION OF END DATE

This is an off-date position funded to 6/30/27 with possible extension contingent upon funding renewal. Minimum Requirements Knowledge equivalent to that which normally would be acquired by completing one or two years of post-bachelor degree work such as a Masters in Student Affairs Administration, Social Work, Developmental Psychology, Educational Psychology or Counselor Education; five to eight years of related and progressively more responsible or expansive work experience in developing and implementing programs, policies and procedures and supervising and coordinating staff; or an equivalent combination of education and experience. Desired Qualifications Desired Qualifications Previous case management experience. Working knowledge of electronic record-keeping systems (Advocate or other case management software). Experience navigating a complex system of multiple offices/partners. Experience supporting students and families in crisis. Experience utilizing trauma-informed models of care. Experience working with graduate students. Strong administrative skills. Strong oral and written communication skills. Training in Mental Health First Aid, continuation of care, suicide prevention, substance abuse, violence risk assessment, de-escalation, restorative justice, and/or active listening.
