Proudly supported by Marquis Health Consulting Services Part-time opportunity available (20 hours) Salary range $25 - $40/hr. At Heritage Hills Rehabilitation and Healthcare Center, we believe that emotional, social, and psychosocial support is essential to helping residents achieve the highest possible quality of life. Guided by our core values of Passion, Respect, and Excellence , the Social Worker supports residents and families by assessing needs, developing care plans, and coordinating services that promote well-being and successful care outcomes. Responsibilities for
Social Worker:
Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations. Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers. Build and maintain positive, supportive relationships with residents and their families. Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments. Develop, implement, and update individualized psychosocial care plans for residents. Participate in care planning meetings and support continuity of care throughout the resident's stay. Provide counseling, emotional support, and resource referrals as needed. Ensure compliance with all applicable facility policies, regulations, and documentation standards. Qualifications for
Social Worker:
Must be able to work full-time hours and maintain a flexible schedule. Bachelor's degree required; master's degree strongly preferred. Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred. Strong knowledge of regulations, policies, and guidelines governing nursing care facilities. Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies. Ability to work effectively in a fast-paced, team-oriented healthcare environment.
