Our Case Manager / Social Worker plays a vital role in making sure every student's social, emotional, and family needs are met β€” so they can show up ready to learn, grow, and thrive. As our program expands to serve more students, this role has never been more essential. πŸŽ“ About the Role As a Case Manager / Social Worker, you will serve as a critical bridge between our students, their families, and the broader support systems that help them succeed. You'll work closely with educators, therapists, and program staff to ensure that each student's individual circumstances are understood, respected, and actively supported. Our students have been diagnosed with emotional disorders, intellectual disabilities, or other developmental challenges. Many come from complex home environments. Your work will directly impact their stability, safety, and long-term trajectory. Due to our expanding enrollment and the addition of new vocational programming, this is a uniquely rewarding time to join our team. πŸ“‹ Key Responsibilities In this role, you will:
  • Develop, coordinate, and monitor individualized case plans for students in collaboration with educators, therapists, and families
  • Serve as the primary point of contact for families, building trusting relationships and ensuring consistent communication
  • Conduct home visits, family meetings, and community outreach as needed to support student well-being
  • Coordinate with external agencies, service providers, and community resources to connect students and families with appropriate supports
  • Assist students in developing social-emotional skills, coping strategies, and self-advocacy tools
  • Support the transition of students into vocational programming by coordinating wrap-around services and community linkages
  • Maintain accurate, timely case documentation and records in compliance with program requirements
  • Participate in multi-disciplinary team meetings, IEP/ISP processes, and program planning discussions
