Job Description

Providence Care is seeking a compassionate and experienced PRN Social Worker to support hospice patients and their families. In this role, you will provide emotional support, resource coordination, and guidance through end-of-life care; working closely with an interdisciplinary team to ensure patients receive care with dignity and respect. What You Will Do Complete psychosocial assessments for patients and families Provide counseling and emotional support Assist with care planning alongside the clinical team Connect families with community resources and services Support bereavement needs as appropriate Educate patients and families on available services and options Maintain accurate and timely documentation Requirements Bachelor's or Master's Degree in Social Work (CSWE accredited) Active South Carolina license ( LMSW, or LISW-CP ) Minimum of 1 year experience in a healthcare setting Hospice or end-of-life experience preferred Strong communication and interpersonal skills Ability to work independently and manage time effectively Providence Care is an Equal Opportunity Employer.
