Job Description

Contract Social Worker position for 13 weeks in or near Rapid City, South Dakota. Provide psychosocial assessments, care planning, counseling, and discharge coordination in long-term care settings. Earn up to $45/hr with premium pay and comprehensive benefits.

Key Responsibilities:

Provide comprehensive psychosocial assessments to residents in long-term care facilities. Develop and implement individualized care plans in collaboration with healthcare teams. Offer counseling and emotional support to residents and their families. Maintain accurate documentation of resident interactions and care plans according to compliance standards. Execute discharge planning and coordinate with community resources for continuity of care.

Qualifications:

Bachelor's degree in Social Work or closely related field. Valid Social Work license (e.g., LCSW, LBSW). CPR certification. First Aid certification. Minimum of 1 year of social work experience in long-term care or related healthcare settings. Ability to work daytime hours (8 a.m. - 5 p.m.). Strong communication skills. Compassionate nature.
