Social Worker

Town Hall Estates Arlington is looking for a Social Worker who is passionate about supporting residents and families through every stage of their journey. This role is at the heart of what we do—helping ensure each resident feels heard, supported, and cared for beyond just their clinical needs. What you'll do: Serve as a key advocate for residents and their families Assist with admissions, discharges, and care planning coordination Provide emotional support and guidance to residents and loved ones Help navigate difficult transitions with empathy and professionalism Coordinate with the interdisciplinary team to ensure holistic care Connect families with resources and services as needed Participate in care plan meetings and help ensure resident goals are met What we're looking for: Someone who genuinely cares and connects well with others Strong communication and interpersonal skills Ability to handle sensitive situations with professionalism and empathy Experience in skilled nursing or healthcare is preferred A team player who is organized, dependable, and proactive
