• Seeking a Social Worker who provides clinical expertise to ensure all patients receive high quality, efficient care. About the facility
• 400-bed full-service hospital
• A leader in Dallas|Fort Worth healthcare

• Provides direct social service and counseling to patients|families
• Assists in locating appropriate resources for post-discharge care
• Participate in multidisciplinary team to coordinate the client's plan
• Cross-trains with team members to serve as back up resource
• Assesses and identifies cases for intervention
• Prioritizes goals and develops plans based on client needs
• Monitors and evaluates outcome of client's plan
• Collaborates with third party payors internal case management|disease management departments as appropriate for discharge planning needs

Requirements:

• Bachelors degree or Masters degree
• Current licensure by Texas State Board of Social Work
• LMSW, LCSW, or LPC preferred
• BLS issued by the American Heart Association
• 1 year social work experience
