The Social Worker will provide essential support and assistance to individuals facing various challenges!

• Duties:

• Conduct assessments to determine clients' needs and develop individualized care plans
• Implement crisis management strategies when necessary
• Collaborate with multidisciplinary teams to ensure comprehensive care for clients
• Deliver hospice care services with empathy and professionalism
• Utilize systems theory in understanding client needs and interventions

Experience:

• LICENSED

  Master's degree in Social Work or related field
• Proficiency in cognitive behavioral therapy and crisis management techniques
• Strong interpersonal skills and the ability to communicate effectively with diverse populations

• Skills:
