401(k) matching Dental insurance Health insurance Vision insurance Licensed Master Social Worker (LMSW) Home Health & Hospice Agency Job Title Licensed Master Social Worker (LMSW) - Part-Time / PRN Job Summary We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our Home Health and Hospice team on a Part-Time or PRN basis. The LMSW will provide psychosocial assessments, counseling, emotional support, resource coordination, and advocacy services to patients and families experiencing illness, disability, end-of-life care, and bereavement. The LMSW will work collaboratively with physicians, nurses, therapists, and other interdisciplinary team members to ensure high-quality patient-centered care. Essential Duties and Responsibilities Conduct comprehensive psychosocial assessments of patients and families. Evaluate social, emotional, financial, environmental, and community support needs. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Provide counseling, crisis intervention, and emotional support to patients and caregivers. Assist patients and families in accessing community resources, financial assistance programs, and support services. Educate patients and families regarding available healthcare, hospice, and community services. Facilitate advance care planning and discussions regarding patient goals and preferences. Participate in interdisciplinary team meetings and contribute to patient care planning. Maintain accurate, timely, and compliant clinical documentation. Coordinate discharge planning and transitions of care when appropriate. Provide bereavement support and grief counseling to families and caregivers. Ensure compliance with federal, state, agency, Medicare, and hospice regulations. Maintain patient confidentiality in accordance with HIPAA requirements. Perform other duties as assigned by the Clinical Director or Administrator. Qualifications Education Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE). Licensure Current Licensed Master Social Worker (LMSW) license in the State of Texas. Current driver's license and reliable transportation. Current automobile liability insurance. Experience Minimum one (1) year of social work experience preferred. Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred. Knowledge of Medicare, Medicaid, and community resources preferred. Skills and Abilities Strong assessment, counseling, and crisis intervention skills. Excellent verbal and written communication abilities. Ability to work independently and as part of an interdisciplinary team. Strong organizational and time-management skills. Compassionate and empathetic approach to patient care. Proficiency with electronic medical records (EMR) systems. Knowledge of hospice philosophy and end-of-life care principles preferred. Physical Requirements Ability to travel to patient homes, facilities, and community locations. Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally. Ability to utilize a computer, tablet, and other office equipment. Work Schedule Part-Time and PRN positions available. Flexible schedule based on patient needs and agency requirements. Some evening or weekend visits may be required.
