Licensed Master Social Worker (LMSW) - Hospice Provide Comfort, Compassion, and Support When Families Need It Most Make a meaningful difference during life's most important moments. Why You'll Love This Role Meaningful one-on-one patient and family relationships Supportive interdisciplinary team environment Flexible scheduling and autonomy in the field Opportunity to make a lasting impact during a critical stage of life Competitive compensation and mileage reimbursement What You'll Do As a Hospice Licensed Master Social Worker (LMSW) , you will: Complete comprehensive psychosocial assessments within five (5) days of a patient's election of hospice services to identify emotional, financial, environmental, and community support needs. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Provide compassionate counseling and emotional support to patients and caregivers coping with serious illness, end-of-life issues, grief, and loss. Assist patients and families with advance directives, funeral planning, discharge planning, and navigating available community resources. Advocate for patients by identifying barriers to care and coordinating appropriate services. Participate in Interdisciplinary Group (IDG) meetings, contributing to patient-centered care planning and continuity of care. Collaborate closely with physicians, nurses, chaplains, and other healthcare professionals to ensure exceptional patient outcomes. Maintain timely, accurate documentation in compliance with agency policies, Medicare Conditions of Participation, and regulatory requirements. Promote dignity, respect, and privacy while providing compassionate, individualized care. Qualifications Required Master's Degree in Social Work (MSW) from an accredited program. Current Texas Licensed Master Social Worker (LMSW) license. At least one (1) year of hospice experience required. Valid Texas Driver's License. Current automobile insurance. American Heart Association (AHA) CPR Certification. Strong communication, counseling, and interpersonal skills. Ability to work independently while collaborating effectively within an interdisciplinary team. Preferred HomeCare HomeBase (HCHB) experience. Knowledge of Medicare Hospice Conditions of Participation. Experience providing end-of-life counseling, bereavement support, and community resource coordination. Why Join Choice Hospice?
