Responsibilities:

Assist patients and families with assessing and addressing social, emotional, and psychological needs Develop and implement individualized care plans to promote patient well-being and independence Collaborate with interdisciplinary teams, including social workers, counselors, and healthcare professionals to provide comprehensive care Provide emotional support and counseling to patients and families, as needed Facilitate patient education and discharge planning to ensure a smooth transition to the next level of care Utilize problem-solving skills to resolve complex social and emotional issues affecting patients and families Maintain accurate and up-to-date records and documentation of patient care

Work Environment:

Work in a fast-paced, dynamic environment with a multidisciplinary team of healthcare professionals Assist patients and families in our short-stay and long-term care units, as well as in our therapy and rehab programs Participate in ongoing training and education to stay current with best practices in social work and patient care

Requirements:

High school diploma or equivalent required; associate's or bachelor's degree in social work or related field preferred 0-1 year of experience in social work or a related field preferred PART_TIME schedule, with flexibility to work varied shifts, including evenings and weekends
