Job Description

Summary The Assistant Director of Comprehensive Homeless Center Programs (ADCHCP) Supervisory Social Worker supports the Director in overseeing all departmental operations. The incumbent assists with strategic direction and provides leadership to staff. The ADCHCP offers a diverse continuum of services across multiple levels of care
  • managed through continuous coordination
  • liaison
  • and consultation with internal program managers
  • Veterans
  • community partners
  • and VA Central Office officials. Duties The ADCHCP Supervisory Social Worker acts as the liaison between the Director CHCP and for the Grant Per Diem (GPO) and Health Care for Homeless Veterans Contract Residential programs (HCHV)

  • and program efficiency In collaboration with the Associate Chief Social Worker for Mental Health

  • and equipment needs The incumbent manages appropriate and non-appropriate accounts
  • authorizing and monitoring expenditures for transitional residences

VANTHCS CHC

  • the incumbent serves as a primary liaison for the GPO
  • HCHV Residential Contract
  • and other CHC programs The incumbent ensures that the GPO
  • HCHV Contract Residential
  • and•when acting for the Director CHC•the CHC programs comply with all relevant regulations•policies•and procedures for the target population and monitors the SAIL measures and medical center performance related to those services The incumbent develops and maintains treatment and documentation standards for CHCP that meet the expectations and requirements of accreditation organizations such as Joint Commission The incumbent performs additional and special assignments that are related to program goals•and the needs of the CHC programs The incumbent identifies high-risk patients and provides crisis intervention services The incumbent provides collaboration•evaluations•and assists with disposition of high-risk CHCP cases to ensure Veteran access to care and provision of services Incumbent completes suicide risk screens and documents assessments as required by policy The incumbent performs additional and special assignments that are related to CHC programs and for other members of the Mental Health Executive Leadership Team with coordination and concurrence of the Director CHCP The incumbent proactively identifies•assesses•and develops comprehensive strategies to mitigate organizational and operational barriers•ensuring continuity and excellence in service delivery The incumbent designs and implements innovative solutions that optimize operational efficiency across the GPO•HUDNASH•HCHV Residential Contract•and other CHC programs•aligning with agency performance goals The incumbent leverages advanced data analytics to track•store•and utilize performance metrics effectively•facilitating the management of Social Work and other discipline workload•quality assurance•and productivity standards within homeless and community based programs The incumbent develops and implements comprehensive training and orientation programs for CHC staff•while also facilitating ongoing professional development activities The incumbent monitors all CHCP-associated contracts•collaborating with the assigned Contracting Report Officer to ensure full compliance The incumbent provides oversight•interventions•and support•to any of the Comprehensive Homeless Center Programs as directed by the
