Job Description
Role:
As a Licensed Social Worker at Avir at Hearne, your primary responsibility will be to assess and address the psycho-social needs of our residents. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans, providing individualized support and services to promote a high quality of life. Your expertise will also be utilized to facilitate group therapy sessions, family support sessions, and other social activities to foster a sense of community and overall well-being.Responsibilities:
Conduct psychosocial assessments and develop individualized care plans to address the physical, emotional, and social needs of our residents Provide individual and group therapy sessions to support residents' mental health and well-being Facilitate family support sessions and other social activities to enhance the overall quality of life for our residents Collaborate with the interdisciplinary team to develop and implement comprehensive care plans Monitor and evaluate the effectiveness of care plans, making adjustments as needed Maintain accurate and timely records of resident interactions and assessmentsRequirements:
Licensed Social Worker (LMSW or LCSW) in the State of Texas Bachelor's degree in Social Work or related field Strong interpersonal and communication skills Ability to work effectively in a fast-paced environment Passion for delivering exceptional patient-centered careBenefits:
