Job Description

River Hills Health and Rehab is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. The Social Worker plays an important role in supporting the emotional, psychosocial, and discharge planning needs of our residents while ensuring quality, person-centered care.
Responsibilities:
Complete psychosocial assessments upon admission and as needed Assist with care planning and participate in interdisciplinary care plan meetings Provide emotional support and counseling to residents and families Coordinate discharge planning and community resources Assist residents and families with advance directives and long-term care planning Address resident concerns and grievances in a timely and professional manner Maintain accurate documentation in accordance with state and federal regulations Support residents' rights and advocate for resident well-being Work collaboratively with nursing staff, therapy, administration, and outside agencies Participate in QAPI and survey readiness activities
Qualifications:
Bachelor's degree in Social Work or related field preferred Previous long-term care or healthcare experience preferred Knowledge of Texas HHSC regulations and long-term care practices preferred Experience with discharge planning, care coordination, and resident advocacy preferred Strong communication, organizational, and problem-solving skills Compassionate, professional, and resident-focused Must be able to pass background screening and meet health requirements
