Salary is determined based on experience. The Care Coordinator is responsible for the coordination, tracking, and support of youth identified as high-risk for or experiencing child sex trafficking through the Children's Advocacy Center for North Texas (CACNT). This includes youth impacted by online exploitation, chronic or repeat runaways, and other trafficking-related vulnerabilities. This role ensures that each assigned youth receives a coordinated, consistent, and informed response across the multidisciplinary team (MDT). The Care Coordinator serves as the central point of accountability for assigned cases, maintaining visibility, supporting case progression, and ensuring no youth falls through the cracks after identification. This position requires strong critical thinking, excellent interpersonal skills, professional ethics, and the ability to remain composed in high-stress situations. Essential Functions Coordinate and track cases involving high-risk youth and child sex trafficking, ensuring continuity from identification through service delivery and case resolution Identify high-risk youth and support a coordinated response, including cases involving online exploitation, chronic runaways, and trafficking indicators Collaborate with multidisciplinary team (MDT) partners, including CPS, law enforcement, prosecution, medical providers, and community agencies, to support coordinated investigations and service delivery Serve as a consistent point of contact for case coordination, maintaining clear and effective communication across partners Participate in case reviews and provide updates on case status, service engagement, and identified needs Facilitate communication and collaboration between partners to support alignment and case progression Accurately document services, case coordination efforts, and outcomes in the agency's case tracking system Track and report data related to high-risk youth and trafficking cases, including required reporting through the Office of the Governor database Support youth and caregivers in navigating systems and accessing appropriate services Develop, coordinate, and deliver trainings and presentations related to high-risk youth and child sex trafficking for multidisciplinary partners and the community Represent CACNT in coalitions, meetings, partner engagements, and community initiatives related to high-risk youth and trafficking Contribute to the development of training materials, resources, and program messaging to support education and awareness efforts Attend trainings and conferences related to high-risk youth and child sex trafficking Other duties as assigned Travel and Expected Hours of Work This position is based in the Lewisville, TX office. The candidate must be willing to work at all offices (Bridgeport, Lewisville, and Denton) when agency needs arise. Working hours are Monday through Friday, 8:00 a.m. to 5 p.m., but work hours may vary and be extended until 9:00 p.m. according to company needs. Education and Training Bachelor's degree in social work, criminal justice, or a related field, required. Experience Minimum 2 years experience working within or alongside multidisciplinary systems (e.g., law enforcement, CPS, CAC, DA's Office) Minimum 2 years experience working in a collaborative, team-based environment Experience with high-risk youth populations (e.g., child sex trafficking, online exploitation, chronic runaways) required Experience working with children and families impacted by trauma or abuse required
