Job Description

Join our dedicated healthcare team as a Licensed Social Worker, where you will play a vital role in providing compassionate, comprehensive social services to diverse patient populations. Your expertise will support individuals and families through emotional, social, and practical challenges, ensuring they receive the highest quality of care. This dynamic position offers an opportunity to make a meaningful difference by facilitating patient assessments, developing personalized care plans, and collaborating with multidisciplinary teams to promote well-being and recovery. Conduct thorough patient assessments to identify social, emotional, and environmental needs across various settings including inpatient, outpatient, hospice, and community care. Develop and implement case management strategies that support patients' mental health, dementia care, and other disabilities. Facilitate group therapy sessions and family counseling to foster supportive environments and improve patient outcomes. Maintain accurate medical documentation in compliance with HIPAA regulations and ensure timely updates of medical records using electronic medical record (EMR) systems. Coordinate intake procedures for new patients and manage crisis intervention efforts effectively during emergencies. Collaborate with healthcare professionals to ensure seamless integration of social work services within treatment plans and post-acute care programs. Extensive experience in family counseling, behavioral health, mental health counseling, and crisis management. Proficiency in clinical research methodologies related to social work practices and healthcare outcomes. Strong hospital experience with knowledge of medical terminology, CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-9/10 systems, and medical documentation standards. Skilled in working with individuals with developmental disabilities, Alzheimer's disease, dementia care, PTSD care, autism spectrum disorders, and geriatrics. Expertise in intake assessments, diagnostic evaluation, case management strategies, and outpatient clinical counseling. Familiarity with EMR systems for data collection and medical records management; experience with inpatient nursing home settings preferred.
Pay:
$55,000.00 - $75,000.00 per year
