ABOUT GUIDING LIGHT HOSPICE
Guiding Light Hospice is a CHAP-accredited, Medicare-certified hospice serving patients and families in San Antonio and the surrounding communities. Hospice social work is not paperwork with a caseload attached. It is sitting in a living room while a family works out what they can and cannot say to each other. It is finding the resource nobody knew existed. It is being the person who names what everyone in the room is already feeling. We're looking for a social worker who can hold that space and still document it well. You'll carry your own caseload, work alongside nurses, aides, chaplains, and our medical director, and have real influence over the plan of care. ━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━WHAT YOU'LL DO
Complete psychosocial assessments at admission and reassess as patient and family needs change Develop and update the social work plan of care in coordination with the interdisciplinary group Provide counseling and emotional support to patients, families, and caregivers Connect families to community resources — financial assistance, placement, benefits, legal and funeral planning Facilitate advance care planning conversations and support decision-making at every stage Assess for safety concerns, caregiver strain, and unmet needs in the home Participate in IDG and contribute the psychosocial perspective to every case discussion Support the bereavement program and follow-up with surviving family members Document accurately and timely in compliance with CHAP standards and Medicare Conditions of Participation ━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━WHO WE'RE LOOKING FOR
You listen more than you talk. You're steady in rooms where nobody else is. You can tell a family a hard truth kindly, and you can write it up cleanly the same day. You understand that documentation is part of the care, not separate from it.REQUIRED
Master's degree in Social Work Current Texas LMSW license in good standing Valid Texas driver's license, reliable transportation, and current auto insurance Ability to travel throughout the city and surrounding areas Working knowledge of Medicare hospice Conditions of Participation and IDG documentation requirements Ability to pass background check, drug screen, and required health screeningsPREFERRED
One year or more of hospice or palliative care experience Experience with hospice EMR documentation Bilingual (English/Spanish) ━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━BENEFITS
