Duties:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Implement crisis management strategies when necessary
- Collaborate with multidisciplinary teams to ensure comprehensive care for clients
- Deliver hospice care services with empathy and professionalism
- Utilize systems theory in understanding client needs and interventions
Experience:
LICENSED
Master's degree in Social Work or related field- Proficiency in cognitive behavioral therapy and crisis management techniques
- Strong interpersonal skills and the ability to communicate effectively with diverse populations
