A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description

Provides professional skills and advanced social services. Responsibilities include performing extended social work program development, case management, clinical research, outreach, patient assessment and evaluation requiring advanced skills and training. Excellent communication skills, both verbally and written; bilingual (Spanish/English) preferred. Ability to work collaboratively and flexibly with a diverse team. Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture. Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. Independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques. Experience using Microsoft Office (Word, Excel, etc.) and data management software.
EDUCATION
Master's degree in Social Work is required.
EXPERIENCE
Three (3) years of professional social work experience is required.
LICENSES/CERTIFICATIONS
License as a Social Worker in Texas (LMSW, LSW) is required. Performs psychosocial assessments, health education, supportive counseling and referrals; provides education on social services and public systems; assists patients in finding resources. Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions. Identifies obstacles to optimal patient care; loss or grief work; risk reduction; and health promotion. Provides clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs as identified. Formulates candid and supportive communication. Coordinates ongoing counseling and support groups. Facilitates effective problem solving and conflict resolution. Participates in program evaluation activities including but not limited to data collection for grants, grant reporting, patient satisfaction surveys, and other activities as needed. Performs all other duties as assigned.
