In this vital role, you will provide comprehensive social work services to diverse patient populations across various care settings, including hospital, hospice, outpatient, and community-based environments. Your expertise will help facilitate patient-centered care, support families through complex health journeys, and coordinate essential community resources. Duties Conduct thorough patient assessments to identify psychosocial needs, medical history, and support systems using clinical research and diagnostic evaluation techniques. Deliver specialized child & family counseling, behavioral therapy, mental health counseling, and clinical counseling tailored to individual patient circumstances. Coordinate discharge planning and home & community care services by collaborating with healthcare providers, community resources, and families to ensure seamless transitions of care. Provide crisis intervention and emergency support utilizing crisis management skills and trauma-informed approaches such as PTSD care and Alzheimer's care. Document all patient interactions accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations and medical documentation standards. Facilitate individual and group therapy sessions, including psychotherapy, motivational interviewing, and applied behavior analysis for patients with developmental disabilities or autism spectrum disorder. Support end-of-life care initiatives by working closely with hospice & palliative medicine teams to deliver compassionate assistance aligned with clinical ethical guidelines. Engage in case management activities such as intake assessments, data collection for research purposes, and ICD coding for accurate medical records classification. Assist with inpatient experience, nursing home experience, assisted living experience, and post-acute care by providing social work expertise in geriatrics, pediatrics, dementia care, and working with individuals with disabilities. Experience Valid licensure as a Master Social Worker (LMSW) with active certification in accordance with state licensing boards. Proven experience in hospital settings or inpatient environments demonstrating familiarity with medical terminology, CPT coding, ICD-9/10 coding systems, and Medicare regulations. Hands-on experience in hospice care or palliative medicine environments emphasizing end-of-life support and advanced care planning. Demonstrated ability to work effectively with children, seniors, individuals with developmental disabilities or autism spectrum disorder (ASD), and those experiencing trauma or PTSD. Strong background in behavioral health practices including cognitive behavioral therapy (CBT), group therapy facilitation, crisis intervention strategies, and working within clinical ethical guidelines. Proficiency in EMR systems for documentation purposes along with knowledge of HIPAA compliance standards related to patient privacy. Experience working with community resources for case management activities such as discharge planning or assisted living placement is highly desirable. Familiarity with medical records management in outpatient or clinic settings along with understanding of ICD coding conventions enhances service delivery quality.
