In this vital role, you will provide comprehensive social work services to patients residing in long-term care facilities, ensuring they receive personalized support that promotes dignity, independence, and quality of life. Your expertise will help coordinate community resources, facilitate discharge planning, and deliver emotional and psychological support to patients and their families. This position offers an exciting opportunity to make a meaningful difference in the lives of aging adults and individuals with complex health needs. Responsibilities Conduct thorough patient assessments to identify social, emotional, and environmental needs, ensuring adherence to social work practice standards and clinical ethical guidelines. Develop individualized care plans that incorporate behavioral therapy, mental health counseling, and crisis intervention strategies tailored to each patient's circumstances. Coordinate with healthcare teams to facilitate discharge planning, including arranging home & community care services, assisted living placements, or hospice & palliative medicine support. Provide clinical counseling sessions—individual or group therapy—to address issues such as addiction, dementia care, PTSD, or developmental disabilities. Maintain accurate medical documentation and medical records while complying with HIPAA regulations to protect patient confidentiality. Collaborate with community resources to connect patients with appropriate services like hospice care, mental health programs, or social support networks. Utilize EMR (Electronic Medical Records) systems efficiently for documentation, data collection, and case management activities. Experience Proven experience working with geriatric patients in long-term care settings or assisted living environments. Strong background in clinical research related to behavioral health or community resource coordination. Hands-on experience with hospice & palliative medicine, end-of-life care, and Alzheimer's or dementia care. Familiarity with medical terminology, ICD coding, Medicare regulations, and hospital or inpatient experience is highly desirable. Demonstrated ability to conduct diagnostic evaluations, crisis management, and motivational interviewing techniques effectively. Experience working with diverse cultural backgrounds and individuals with disabilities—including autism spectrum disorder—is preferred. Prior involvement in home & community care programs or outpatient clinics focusing on mental health counseling or behavioral health is advantageous.
