Job Description

Summary This position is eligible for the Education Debt Reduction Program (EDRP)
  • a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval

  • and engaging in healthcare and recovery services Provide case management and care coordination to facilitate appropriate delivery of healthcare services Conduct face-to-face meetings with Veterans in their homes or community settings as clinically appropriate Collaborate with interdisciplinary teams

  • and community partners to ensure coordinated care Maintain a network of community resources and referral options to support Veteran stability and recovery Advocate for Veterans within VA systems and external service networks Utilizes working knowledge of community-based housing

Pay:
Competitive salary and regular salary increases
Paid Time Off:
37-50 days of annual paid time offer per year (13-26 days of annual leave
