What You'll Do Complete psychosocial assessments for home health patients Identify social, emotional, environmental, and financial needs Connect patients and families with community resources and support services Assist with long-term planning and discharge planning Provide counseling, education, and emotional support to patients and caregivers Collaborate with physicians and interdisciplinary team members Maintain timely and accurate documentation Qualifications Active Virginia Licensed Master Social Worker (LMSW) License Required Master's Degree in Social Work from an Accredited Program Valid Driver's License and Reliable Transportation Strong Communication, Assessment, and Documentation Skills Home Health or Healthcare Experience Preferred but Not Required Passion for Helping Patients and Families Overcome Challenges Why Human Touch Home Health?
