Full-time Hospice / Home Health Social Worker located near Spokane, WA. Provide compassionate care and support for patients and families in hospice and home health settings.

Key Responsibilities:

Conduct psychosocial assessments for hospice and home health patients Provide emotional support, counseling, and crisis intervention for patients and families Assist patients and caregivers with advance care planning and end‑of‑life support Connect patients with community resources, including Medicaid assistance, Meals on Wheels, medical transportation, caregiver support, utility and housing assistance Coordinate respite care services and hospice transfers as needed Collaborate with nurses, physicians, chaplains, and interdisciplinary team members Advocate for patient rights, dignity, and quality of life Maintain accurate documentation in compliance with hospice and home health regulations

Qualifications:

Bachelor's or Master's degree in Social Work from an accredited program Active Idaho Social Work license Licensed Master Social Worker (LMSW) required Knowledge of community resources, Medicaid, and social service systems Strong communication, organization, and time‑management skills Valid driver's license with ability to travel to patient homes

Preferred Skills:

Experience in hospice, home health, medical social work, or healthcare setting
