Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description

Join our non-profit, community based Hospice and Palliative Care team to truly make the difference in the life of others! Regional Hospice Services is seeking a compassionate individual to join our dynamic, interdisciplinary team that works to support patient and family centered goals. You will provide social work support to our patients and families in their home setting as well as provide grief support to those in our local community. A bachelor's degree in social work or related field and social work licensure in the state of Wisconsin is required. The ideal candidate will have an active knowledge of and association with social services in the Ashland community and surrounding area. Position is flexible and approximately 20 hours per week. Must be able to work independently. Regional travel required. Benefits include flexible scheduling, paid time off, annuity with employer matching funds, cell phone stipend, travel time and mileage reimbursement.
