Shift Details:
Minimum requirement one weekend per month Saturday in person coverage with Sunday pager coverage. Weekday shifts available as interested.
Job Summary:
Reporting to the Supervisor Case Management, the Social Worker is responsible for coordinating the care of designated patients and families during a patient's hospital admission. The Social Worker coordinates the discharge plan from pre-admission through discharge follow-up. The Social Worker provides psychosocial assessment and updates that address the patient's evolving needs. The Social Worker establishes a plan of care that provides maximum benefit to patients and their families. The goal of social work services is to be an advocate for patients across the continuum, connecting them with the necessary resources to ensure safe and effective care. Other duties as assigned.
EXPERIENCE DESCRIPTION
A minimum of 1-2 years of experience in an acute care setting is preferred.
EDUCATION DESCRIPTION
Master's degree in Social Work is required.
SPECIAL SKILLS DESCRIPTION
Demonstrated ability to work effectively in a team environment and interact with physicians, other professional disciplines, external agencies and organizations. Requires strong computer skills. Medical social work, including discharge planning preferred. Interpersonal skills necessary to obtain information from patients/ families, and interact effectively with patients, families and hospital personnel. Analytical skills necessary to assess patient's needs, develop associated discharge planning and develop solutions to problems concerning patients mental and physical well-being.
LICENSURE DESCRIPTION
Current State of Wisconsin license in social work is required.
