Job Description

Overview We are seeking a dedicated and compassionate Hospice Social Worker to provide per-visit patient support in the Phoenix West Valley area. This vital role involves delivering comprehensive psychosocial assessments, emotional support, and resource coordination to individuals facing end-of-life challenges. The ideal candidate will possess a strong background in social work practice within hospice and palliative care settings, demonstrating excellent communication skills and cultural sensitivity. This position offers an opportunity to make a meaningful difference in patients' lives while working within a collaborative healthcare team committed to compassionate care. Duties Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and practical needs related to end-of-life care. Provide individual, family, and group counseling utilizing clinical techniques such as motivational interviewing, behavioral therapy, and crisis intervention. Coordinate community resources and support services to enhance patient quality of life, including assistance with housing, financial aid, and spiritual support. Document all patient interactions accurately within electronic medical records (EMR) systems, adhering to medical documentation standards and HIPAA regulations. Collaborate with interdisciplinary team members—including nurses, physicians, chaplains, and therapists—to develop comprehensive care plans aligned with clinical ethical guidelines. Educate patients and families about hospice care options, medical terminology related to palliative medicine, and discharge planning procedures. Engage in ongoing professional development through clinical research participation and staying current with hospice & palliative medicine best practices. Skills Extensive experience in hospice care, medical social work, or palliative medicine with a focus on end-of-life issues. Proficiency in adult & family counseling, behavioral health interventions, and mental health counseling techniques. Knowledge of medical terminology and Medicare regulations relevant to hospice services. Strong skills in data collection, case management, diagnostic evaluation, and client communication across diverse populations. Familiarity with EMR systems for documentation; experience working with hospital or inpatient settings is advantageous. Ability to coordinate community resources effectively while demonstrating cultural competence when working with individuals from diverse backgrounds. Certification in basic life support (BLS) or CPR; experience in crisis management and assisted living or nursing home environments is preferred.
