- Provide oversight and direction to Social Services Departments across multiple facilities.
- Ensure consistent implementation of social service policies, procedures, and best practices.
- Conduct regular site visits to assess department performance, patient outcomes, and operational efficiency.
- Partner with facility leadership teams to support quality improvement initiatives and patient satisfaction goals. Regulatory Compliance & Quality Assurance
- Ensure compliance with all federal, state, and local healthcare regulations, including CMS and Department of Health standards.
- Monitor survey readiness and participate in regulatory audits and inspections.
- Review psychosocial documentation, care plans, and discharge planning practices for compliance and accuracy.
- Develop and implement corrective action plans as needed. Staff Development & Mentorship
- Recruit, train, mentor, and support facility Social Services Directors and social work staff.
- Provide leadership coaching and ongoing education related to psychosocial care, documentation, and regulatory changes.
- Assist facilities with staffing coverage and support during vacancies or leadership transitions.
- Promote a culture of collaboration, accountability, and professional growth. Clinical Oversight
- Provide consultation and guidance on complex psychosocial cases, crisis interventions, behavioral health concerns, and family dynamics.
- Support interdisciplinary care planning and patient advocacy efforts.
- Ensure appropriate discharge planning processes are in place to support safe transitions of care.
- Collaborate with nursing, therapy, admissions, and medical teams to improve patient outcomes and reduce avoidable readmissions. Performance & Reporting
- Analyze regional social services metrics and identify trends or opportunities for improvement.
- Prepare reports for executive leadership regarding compliance, staffing, quality measures, and operational performance.
- Participate in strategic planning initiatives related to clinical and psychosocial services.
Qualifications Education:
- LCSW Licensure
- Active Licensed Social Worker (LSW) license in the state of practice required
- Additional state licensure may be required based on regional coverage
Experience:
- Minimum of 5 years of progressive social work experience in healthcare, long-term care, rehabilitation, or hospital settings
- Minimum of 2 years of leadership or multi-site management experience preferred
- Strong knowledge of CMS regulations, discharge planning, behavioral health, and long-term care operations
- Required Skills & Competencies

- Excellent communication and conflict resolution abilities
- Knowledge of gerontology and psychosocial care practices
- Experience with healthcare compliance and regulatory surveys
- Crisis intervention and case management expertise
- Strong organizational and problem-solving skills
- Ability to travel regularly throughout assigned region
Pay:
$50.00- $55.
