Job Description

Job Summary We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will provide essential support to patients and their families, assisting them in navigating the complexities of healthcare systems while addressing emotional, social, and financial challenges. This role requires a strong understanding of clinical counseling and patient assessment, along with the ability to work collaboratively with multidisciplinary teams to ensure comprehensive care for patients. Duties Conduct thorough patient assessments to identify social, emotional, and environmental factors impacting health and well-being. Provide clinical counseling services to individuals, families, and groups, including child family counseling and group therapy. Develop and implement care plans that address the specific needs of patients, particularly those with dementia or requiring crisis intervention. Ensure compliance with HIPAA regulations while maintaining confidentiality and privacy of patient information. Collaborate with healthcare professionals to coordinate care and resources for patients, advocating for their needs within the healthcare system. Utilize ICD-10 and ICD-9 coding systems for accurate documentation and reporting of patient diagnoses and treatment plans. Participate in ongoing professional development to stay current with best practices in medical social work. Experience A Master's degree in Social Work (MSW) from an accredited institution is required. Previous experience in a healthcare setting or related field is preferred. Proficiency in crisis intervention techniques and patient assessment methodologies is essential. Familiarity with dementia care practices is a plus. Strong communication skills, both verbal and written, are necessary for effective interaction with patients, families, and healthcare teams.
