(CVIH) is seeking compassionate, resourceful candidates for the position of Licensed Behavioral Health Clinician & Case Manager in Clovis, CA. This position provides support to patients and families through assessment of psychosocial needs, care coordination, and connection to community resources. The role's primary focus is on addressing basic needs, including housing, food security, transportation, and benefits navigation. A secondary responsibility includes delivering therapy services, such as brief counseling, psychoeducation, or referrals for ongoing mental health treatment. The incumbent holds a "Safety Sensitive" position and is responsible for HIPAA compliance.
PRIMARY RESPONSIBILITIES
Resource Navigation & Case Management (Primary Focus) Assess & triage patients for needs related to housing, food, transportation, financial assistance, employment, and other social determinants of health and coordination of care within CVIH and the community. Connect patients to community resources, government programs, and supportive services. Provide case management support, including follow-up and advocacy to ensure continuity of care. Maintain an updated directory of local and regional resources. Collaborate with multidisciplinary care teams to integrate psychosocial support into treatment planning. Therapy & Clinical Support (Secondary Focus) Conduct brief, solution-focused therapy or supportive counseling as appropriate. Provide psychoeducation and supportive interventions to individuals, families, and groups. Refer patients requiring long-term or specialized therapy to appropriate providers. Maintain accurate and timely clinical documentation in accordance with agency, state, and federal standards. General Duties Participate in team meetings, case conferences, and interdepartmental collaboration. Support the clinic's mission of culturally responsive and community-centered care. Engage in professional development, supervision, and continuing education as required.
Requirements:
1. Master's Degree in Social Work (MSW) from an accredited program. 2. Current California LCSW, or equivalent licensure/registration. 3. Minimum of two years' experience in community resource navigation, case management, or related role is preferred. 4. Familiarity with clinical interventions, trauma-informed care, and culturally responsive practice. 5. Strong communication, organizational, and advocacy skills. 6. Ability to work collaboratively with multidisciplinary teams and diverse patient populations. 7. Valid CPR Certificate preferred. 8. Clearance from the National Practitioner Data Bank. 9. Knowledge of office skills, typing, filing, medical terminology, and computer literate for word processing. 10. A valid California Driver's License and clean driving record with the ability to be insured under CVIH's automobile insurance. 11. An awareness and keen appreciation of Indian traditions, customs, and socioeconomic needs. Ability to meet and deal effectively with Indian health organizations consisting of numerous tribes with varied cultures. Tact, courtesy, discretion, resourcefulness, and good judgment in handling functions of a sensitive nature. 12.
