APPOXIMATE PAY RANGE
$40.91 - $50.31 POSITION SUMMARY Under general supervision assesses patient and family Psycho-Social and discharge planning issues relevant to medical treatment. Provides crisis intervention, emotional support, resource information, discharge planning, and legal reporting. Arranges case conferences, facilitates Bio-ethical consultations. Complies with ethical and professional standards of conduct as set forth by the national Association of Social Workers. Is an advocate of patient?s rights. Provides information and referral services for general public as well as other professionals employed at hospital; Provides Advance Directives information to patients, family and friends and assists with completion of Living Wills, Medical Power of Attorney, Health Care Surrogate or any other appropriate decision making documents as specified by state and federal law including but not limited to committee ship or Guardianship judgements. Assists with adoption process. Provides all call services for acute care and Emergency Department. Able to adapt to constantly changing healthcare environment by recognizing opportunities for improvement and embracing change to bring about improvements.
ESSENTIAL DUTIES 1.
Effectively facilities and documents patients that are admitted/returning to Nursing Home Facilities. 2. Keeps abreast of current trends and developments as related to Case Management/Social Services, discharge planning, and home health. 3. Performs psychosocial assessments and discharge planning for high risk patients. 4. Participates in patient care planning responsive to age specific needs. 5. Refers patients to appropriate community resources. 6. Provides crisis intervention and emotional support to patients and families on bio-psychosocial issues. 7. Coordinates patient/family physician conferences. 8. Provides consultation to physicians and hospital staff re; psychosocial issues, bioethical issue and discharge problems. 9. Develops and maintains working relationships with community agencies. 10. Participates in planning the social work component required in selected hospital programs.
Required Qualification:
EDUCATION, EXPERIENCE, TRAINING 1.
Current California LCSW preferred or LCSW eligible within 3 years of employment preferred. 2. Masters degree in Social Work or related field of studies required. 3. Current Basic Life Support (BLS) required. 4. Sexual Harassment/ Bullying Prevention Certification required. (1 month from hire) 5. Minimum one year acute care hospital experience preferred. 6. Must possess excellent interpersonal skills, assertive, highly motivated and exercise independent thinking, judgement, and decision-making. Ability to withstand long hours and function under stress.
