Master's Degree in Social Work (MSW) Minimum 1 full year of recent Acute Care Inpatient Hospital Social Work / Case Management experience Recent hospital experience within the last 3 months Previous Travel Assignment experience required ( No First-Time Travelers ) Experience with Epic EMR ( Required- Hard Stop )
KEY RESPONSIBILITIES
Complete psychosocial assessments within required timelines. Develop individualized plans of care for hospitalized patients. Coordinate post-acute services including home health, DME, rehabilitation, and skilled nursing placement. Collaborate with physicians, nursing staff, utilization review, insurance case managers, and community agencies. Maintain timely documentation in Epic EMR . Manage an average caseload of 20-23 adult patients .PREFERRED QUALIFICATION
LCSW- preferred BLS Certification Trauma Level I or II Hospital experience Experience with: Care Coordination | Utilization Review | Discharge Planning | Prior Authorizations | DME Coordination | Community Resource Navigation | Medicare & Medicaid Programs | Adult Protective Services | Advance Directives
COMPENSATION
Local W2: $63.50/hr on W2 = $2,540.00/WK. Travel W2: $25.00/hr on W2 + Stipends (Meals:
$70/day |Housing:
