- Part-time, Full-time We are looking to have a full time or part time Licensed Clinical Social Worker join our team at Garfield Health Center.
Essential Duties and Responsibilities:
Individual and group psychotherapy Develop diagnosis and treatment plans that are integrated with the medical treatment plan Provide intervention therapy for mild-to-moderate mental health conditions Educate and assist the patient/family in understanding and adjusting to the diagnosis/disability, treatment plan, and medical recommendations Provide age-appropriate and culturally sensitive counseling and individual/family treatment as well as assistance in family life crisis situations Prepare and maintain all pertinent records, statistics, and progress notes Work with medical providers for an integrated approach to care during warm hand-off Perform all other related duties as appropriate and/or assignedQualifications:
Masters in Social Work minimum requirement. Licensure as a Licensed Clinical Social Worker by the California Board of Behavioral Sciences minimum requirement. Certified substance use disorder (preferred) Bilingual Chinese speaking (required)Job Types:
