Job Description

Job Summary The Hospice Social Worker provides compassionate, comprehensive care to patients and their families during the end-of-life journey. This role involves conducting psychosocial assessments, developing individualized care plans, and offering emotional support to patients and families facing terminal illness. The Social Worker/Bereavement Coordinator will also manage grief counseling and bereavement services for families both before and after the death of a loved one. This position requires strong collaboration with an interdisciplinary healthcare team, as well as the ability to connect patients and families with community resources and practical support. A bilingual (English/Spanish) professional is highly preferred to ensure culturally competent care and effective communication with a diverse patient population. Responsibilities Conduct thorough psychosocial assessments for patients and families to identify emotional, social, and financial needs. Provide emotional support to patients and families facing terminal illness, helping them manage grief, fear, and anxiety. Offer counseling services, both individual and family-oriented, to help patients and their loved ones cope with the challenges of end-of-life care. Advocate for the patient's and family's wishes in relation to care, ensuring they receive appropriate services and support. Assist with creating and maintaining care plans tailored to the patient's and family's unique needs. Develop and implement bereavement programs and services to support families before and after the death of a loved one. Facilitate support groups for individuals coping with grief and loss. Provide ongoing counseling to families who are navigating the bereavement process, offering emotional and psychological support during the grieving period. Offer education on grief and loss, including normal grief reactions and how to seek additional support if needed. Collaborate with doctors, nurses, chaplains, and other healthcare professionals to provide comprehensive, holistic care to the patient and family. Participate in interdisciplinary team meetings to discuss the care plan, report progress, and address family concerns. Maintain accurate and confidential documentation of patient and family interactions, assessments, and care plans. Prepare regular reports for team meetings, detailing updates on patients' emotional and psychosocial status. Ensure compliance with HIPAA and all applicable regulatory and ethical standards in documenting and sharing information. Skills Strong background in social work principles and practices. Proficiency in crisis intervention techniques and behavioral health support. Ability to assess the emotional, social, and psychological needs of patients and families. Providing empathetic, non-judgmental communication to individuals dealing with grief. Excellent communication skills, both verbal and written, for effective client interaction. Working effectively within a multidisciplinary team (doctors, nurses, chaplains, volunteers) to create a cohesive care plan.
