Cedar Park Group is seeking a Licensed Clinical Social Worker in Salinas, Puerto Rico. This is a non-clinical administrative position responsible for medical readiness determinations, case management, records management, and coordination of care. Required Qualifications Master of Social Work (MSW) from a CSWE-accredited institution. Current, active, valid, unrestricted clinical social worker license. Essential Duties Perform initial and follow-up behavioral health screening evaluations using clinical interviews, medical documentation, and collateral information. Determine provisional diagnoses from available medical documentation. Identify treatment plans consistent with regulations and medical readiness requirements. Manage a behavioral health caseload of up to 300 patients. Develop and maintain a network of licensed behavioral health providers. Coordinate referrals with providers, Treatment Facilities, and the VA. Prepare written recommendations regarding Line-of-Duty and Incapacitation Pay status related to behavioral health conditions.
