Job Description

Medical Social Worker I - Discharge Planning / Case Management (Behavioral Health)
Location:
South San Francisco, CA 94080
Schedule:
Full-time, 40 hours/week | Monday-Friday, 8:00 AM-4:30
PM Required Qualifications:
Master of Social Work (MSW) from a CSWE-accredited program. Current California social work license (e.g., LCSW, LMSW, or ASW under supervision, as applicable). Current BLS (Basic Life Support) certification. Experience in hospital-based discharge planning and case management preferred; new graduates with strong clinical placements considered. Strong knowledge of behavioral health conditions , community resources, and post-acute care options. Excellent communication, documentation, and interdisciplinary collaboration skills.
Preferred Qualifications:
Prior experience in an acute care hospital or behavioral health setting. Familiarity with utilization review, and care management processes. Bilingual skills (e.g., Spanish, Cantonese, Tagalog) to serve a diverse patient population.
Pay:
$50.65 - $55.00 per hour Expected hours: 40.0 per week
