Job Description

Job Overview We are seeking a compassionate and dedicated Medical Social Worker specializing in hospice care to join our interdisciplinary team. In this vital role, you will provide emotional support, counseling, and case management services to patients facing life-limiting illnesses and their families. Your expertise will help enhance quality of life, facilitate effective communication with healthcare providers, and ensure comprehensive psychosocial support tailored to each patient's unique needs. Duties Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems. Provide individual and group counseling sessions utilizing techniques such as behavioral therapy, motivational interviewing, and psychotherapy to address emotional distress, grief, or adjustment issues. Collaborate with medical teams to develop personalized care plans that incorporate social work interventions and support services. Manage intake processes for new patients, ensuring accurate documentation of medical records while adhering to HIPAA regulations for patient privacy and confidentiality. Facilitate crisis intervention when urgent emotional or behavioral issues arise, including working with individuals experiencing PTSD or dementia-related behaviors. Coordinate community resources, including assisted living facilities, home health services, and specialized programs like Alzheimer's or autism care, to support patient needs beyond clinical settings. Maintain detailed medical documentation within electronic medical record (EMR) systems, ensuring compliance with ICD coding standards such as ICD-10 and CPT coding where applicable. Participate in clinical research initiatives related to hospice care or behavioral health to advance best practices and improve patient outcomes. Qualifications Master's degree (MSW) required from an accredited program. Valid licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent in the relevant state or region. Proven experience in hospice care, inpatient settings, nursing homes, or home & community care environments is highly desirable. Strong knowledge of medical terminology, medical documentation standards, ICD coding (ICD-9/ICD-10), and HIPAA regulations. Demonstrated skills in child & family counseling, behavioral health therapy—including cognitive behavioral therapy—and crisis management. Experience working with diverse populations such as seniors with geriatrics needs, individuals with disabilities or developmental disabilities, Alzheimer's disease patients, PTSD care recipients, and those requiring post-acute or outpatient care. Familiarity with EMR systems and data collection methods essential for tracking patient progress and clinical research projects. Excellent communication skills for engaging patients and families effectively while providing compassionate mental health counseling and case management services.
