ESSENTIAL FUNCTIONS
An employee in this position may be called upon to do any or all the following essential functions. These examples do not include all the functions which the employee may be expected to perform. 1. Develops, implements, and monitors service/treatment interventions that provide a variety of services to meet patient/family needs. 2. May serve on department and hospital-wide committees as directed. 3. Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment. 4. Develops and maintains relationships with peers within and outside of the department as well as community resources. 5. Conducts initial needs assessment to gather information on the patient/family's medical, social, and financial history. 6. Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals. 7. Facilitates staffing, care conferences, etc. to coordinate care between the treatment team. 8. Compiles and prepares case records, reports, and documents that comply with state and federal standards in providing case notes, treatment plans, and evaluations. 9. May provide clinical supervision and support to clinical social work interns. 10. Provides emotional support to multidisciplinary team members in the context of patient care. 11. Participate in on call and coverage of other CSW team members.SCOPE & LEVEL

Work assignment is performed within an established framework under general instructions but requires simultaneous coordination of assigned functions or projects in various stages of completion.Decision Making:
Decisions or recommendations on non-standardized situations are limited to relating organizational policies to specific cases. Brings non-routine issues to supervisor. Problems that are not covered by guidelines or that are without precedent are taken up with the supervisor.Communications:
Contacts with team members, clients or the public where explanatory or interpretive information is exchanged, gathered, or presented and some degree of discretion and judgment are required within the parameters of the job function.Supervision Received:
Performs work with minimal supervisory oversight. Under supervision, the employee receives assignments and is expected to carry them through to completion with independence. Work is reviewed for adherence to instructions, accuracy, completeness, and conformance to standard practice or precedent. Frequency of supervision is determined by licensure status.. Minimum Qualifications Degrees Masters Area of Study Social Work Experience One (1) year of post-Master's full-time work experience providing psycho-social assessments and interventions to patients and families Licenses & Certifications Licensed Social Worker (LSW) in the state of Colorado (CO) Schedule/Shift 30hrs FT Thursday- Saturday 1p-11p Salary Information Pay is dependent on applicant's relevant experience.
