Job Description
Lee Health seeks an experienced Medical Social Worker to deliver compassionate, patient-centered care across our Southwest Florida health system. In this role, you will complete psychosocial assessments, provide counseling and crisis intervention, and coordinate discharge planning to ensure safe transitions of care. You will connect patients and families with community resources, collaborate closely with interdisciplinary teams, and advocate for access to needed services. Responsibilities Provide psychosocial assessments for patients and families in medical settings Develop and implement care plans addressing social, emotional, and resource needs Coordinate discharge planning and connect patients with community resources Collaborate with physicians, nurses, and interdisciplinary teams on patient care Offer crisis intervention, counseling, and support during acute illness or hospitalization Advocate for patients' rights, access to care, and continuity of services Educate patients and families on coping strategies, treatment plans, and support options Maintain accurate documentation in the electronic health record and comply with regulations Participate in quality improvement and population health initiatives Support Lee Health's mission, safety standards, and continuous learning culture Required Skills Clinical psychosocial assessment Discharge planning and care coordination Crisis intervention Brief counseling and supportive therapy Knowledge of community and social service resources Electronic Health Record (EHR) documentation Interdisciplinary team collaboration Patient and family education Case management in healthcare settings Regulatory and compliance knowledge (HIPAA, CMS)
