Care Team Role - Medical Social Worker (MSW)
Supervisor:
Clinical Manager (DON)
Job Purpose:
An MSW provides medical social services within the scope of MSW practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families.
Qualifications:
A Masters degree from a program accredited by the Council on Social Work education. A current license in FL. A minimum of one (1) year experience in Social Work, preferably in the homecare setting. Must have a criminal background check. Must have current CPR certification.
Florida Requirements:
The Registered Nurse functions as the case manager for patients receiving nursing services or nursing in combination with therapy services. The Registered Nurse is responsible for coordination of care , implementation of the plan of care , and maintenance of the clinical record for assigned patients. The Registered Nurse provides clinical supervision of Licensed Practical Nurses, Home Health Aides (HHAs) and Certified Nursing Assistants (CNAs) involved in the patient's care, consistent with Florida law and agency policy.
Compensation:
PRN or Part-Time $35 - $55/Hourly Based on
Experience Responsibilities:
Receives assigned patient referrals from clinical leadership and prepares assessment tools and documentation required for the initial visit. Obtains all pertinent medical history from patient, family or significant others. Performs home safety check and environmental assessment of the patient's home environment. Performs the socio-psychological evaluation of the support systems available to the patient and documents necessary emergency contacts etc. Provides medical social work services addressing psychosocial, emotional, environmental, and financial factors that impact the patient's ability to achieve optimal health outcomes. Conducts medically necessary skilled medical social work visits in accordance with the physician-ordered plan of care and documents findings and interventions in the clinical record. Provides supportive casework designed to restore patient's to their optimum level of social adjustment. This includes assisting patients and their families to understand, accept and follow medical recommendations. Assists patients utilize the resources of their families and the community at large. Resource utilization may include referring the patients to community resources or acting as an intermediary on behalf of the patients with other health and welfare agencies. Effectively communicates with patient and family. Effectively communicates with other disciplines in the case (case conferencing) to effectively and appropriately problem solve as situations arise. Communicates effectively with the Director of Nursing or Nursing Supervisor scheduled visits planned and changes to the schedule on a weekly basis. Manages caseload scheduling in coordination with clinical leadership and communicates planned visits and schedule changes in a timely manner. Communicates with the patient's physician or allowed practitioner regarding psychosocial findings, barriers to care, and recommended interventions within scope of practice. Communicates in the case conferencing sessions to establish best practices for the individual patient's needs. Submits complete and timely documentation in accordance with Agency documentation standards and timelines. Coordinates Community Services that may be available to the patient to assist in safe home care needs. Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office. Ensures that services provided are medically necessary, ordered, documented appropriately, and consistent with payer requirements. Participates in the QA Committee. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event's occurrence.

Must be able to travel to prospective patient's place of residence. Must be able to hear and effectively communicate in English. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, and the ability to adjust focus.
