Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Job Duties :
•Conduct comprehensive psychosocial assessments to evaluate clients' mental, emotional, behavioural, and social needs. Develop, implement, and monitor individualized treatment plans in collaboration with patients, families, and interdisciplinary healthcare teams. Provide individual, family, and group psychotherapy using evidence-based therapeutic interventions. Perform crisis intervention, suicide risk assessments, and safety planning for clients experiencing acute mental health concerns. Coordinate referrals to community resources, support services, housing, financial assistance, and other healthcare providers as needed. Maintain accurate, timely, and confidential clinical documentation in compliance with HIPAA, state regulations, and organizational policies. Educate clients and families on coping strategies, disease management, available resources, and discharge planning to promote long-term well-being. Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure continuity of care and improve patient outcomes.
