+ Coordinate the interdisciplinary plan of care across the continuum from admission to discharge while integrating patient and family preferences

+ Monitor patient care through assessments, evaluation, and record reviews while serving as a resource for documentation, service appropriateness, and post-discharge options

+ Advocate for necessary resources and barrier removal while maintaining ongoing dialogue with supervisors and providers to execute care plans

+ Provide clinical oversight of other associates as needed What minimum requirements you'll need Licensure / Certification /

Registration:
+ One or more of the following required:

+ Clinical Counselor obtained prior to hire date or job transfer date.

+ Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.

+ Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.

+ Case Manager credentialed from the Commission for Case Manager Certification ( CCMC ) preferred.

+ Case Manager credentialed from the American Case Management Association ( ACMA ) preferred.

Education:

+ Master's degree in Social Work required.

Work Experience:

+ 3 years of experience preferred What additional preferences we're seeking

+ Hospital care management experience needed

+ Previous Behavioral Health experience desired
