Pay Range:
$65,000
  • $80,000, Based on experience

KY Education:
Masters' Degree from an accredited school of social work required. Strong communication and customer service skills Ability to follow care plans and resident orders Long-term care experience (preferred) Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Must have a valid driver's license and have means of transportation. Must clear background check. What You'll Be Responsible For Serve as a contributing member to the PACE Interdisciplinary Team (IDT). Provide direct, hands-on clinical care to residents, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility Support residents' physical and emotional well-being by building meaningful connections and delivering compassionate, person-centered care Monitor and report changes in residents' condition to the nursing team to ensure timely and appropriate care Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards About Us Senior Community Care of Kentucky PACE (Northern) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant's needs. Located in the welcoming community of Florence , PACE is part of the greater Northern Kentucky region. Just minutes from downtown Cincinnati, our location offers access to a vibrant riverfront city with dining, arts, and cultural attractions.
