Job Description
Position:
Case Manager. Must possess a Master's Degree in a Social Service field.Nature of Work:
The Case Manager will be responsible for assisting with the daily implementation of social services and counseling in the residential programs.Line of Supervision:
Directly responsible to the Social Services Supervisor.Duties and Responsibilities:
1. To provide counseling and/or therapy services to residents and families. 2. To co-facilitate all group therapy services to residents and families. 3. To document the provisions of all social services in each resident's record. 4. To assist in the preparation of quarterly reports. 5. To participate in the development of a behavior modification plan appropriate for each resident. 6. To participate in the training of staff in the behavior modification plan and monitor the plan on a daily basis. 7. To participate in the staffing of each resident's Individual Service Plan. 8. To maintain a social service record on each resident. 9. To coordinate all social services provided to residents by outside agencies such as mental health and substance abuse treatment as indicated. 10. To prepare social histories on residents and families as needed for the purpose of psychological/psychiatric evaluations. 11. To perform any service the Social Services Supervisor may request as it pertains to the care of residents or the proper administration of the social service programs required by the facility or regulatory agencies. 12. To assist resident in finding employment if applicable. 13. To perform other such duties and accept other such responsibilities as required by the Director.Qualifications:
The Case Manager must: 1. Have a Master's degree from an accredited college or university. 2. Have a minimum of two years experience in direct practice in the social work/mental health or related field. Have a valid Louisiana Class E driver's license.' 'Benefit Conditions:
