Resource guidance. Advocacy. Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a part-time Social Worker (MSW) to provide emotional support, counseling, advocacy, and community resource coordination for hospice patients and their loved ones while collaborating closely with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care. This role could turn into a full-time opportunity as our census continues to rise.
