Resource guidance. Advocacy. Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a part-time Social Worker (MSW) to provide emotional support, counseling, advocacy, and community resource coordination for hospice patients and their loved ones while collaborating closely with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care. This role could turn into a full-time opportunity as our census continues to rise. Schedule, Hours & Territory Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM Patients may be located in Macomb, Oakland, Wayne, Livingston, and Washtenaw Counties (mileage reimbursement is applicable) What We Offer Meaningful role supporting patients and families emotionally and socially Strong interdisciplinary team collaboration Additional compensation for on-call participation Mission-driven hospice environment Key Responsibilities Conduct psychosocial assessments Provide counseling and emotional support to patients and families Assist with advanced care planning and coordination of community resources Participate in interdisciplinary team meetings Document services accurately and timely in the

(Homecare Homebase) Master's degree in Social Work preferred Bachelor's degree in Social Work accepted with required oversight Hospice experience preferred Experience using HCHB (Homecare Homebase) preferred Field Role Requirements Active driver's license Ability to pass a criminal background check Ability to pass a Motor Vehicle Record (MVR) check Current TB testing in accordance with agency policy Current CPR certification as required for the role
