Provide behavioral health assessments, counseling, and care coordination. Collaborate with primary care providers within an integrated care model. Participate in school and community outreach programs. Maintain accurate clinical documentation in the electronic health record.
Qualifications:
MSW required, Missouri LCSW license or eligibility. Experience in clinical or medical settings preferred. Strong communication and teamwork skills.
Schedule & Benefits:
